Monthly Communication Form

Social connections are important to our mental health especially for people who live alone or may be suffering physically or emotionally. In order to make sure that I am staying in touch with family & friends in the coming months, I have decided to put together a “Communication Form” to ensure that I am reaching out to everyone on a regular basis and no one “slips through the cracks”.
